Please read these Terms of Service carefully before using our services. By scheduling an appointment or receiving services at Medspa Bella, PLLC, you agree to be bound by these terms.
Services
Medspa Bella, PLLC provides medical aesthetic services under the supervision of a licensed physician. All procedures are performed by qualified medical professionals in accordance with Florida law.
Appointment Policy
Appointments are required for all services. We recommend scheduling at least one week in advance to ensure availability.
Cancellation Policy
We require 24 hours notice for appointment cancellations or rescheduling. Late cancellations or no-shows may be subject to a cancellation fee equal to 50% of the scheduled service cost. We understand that emergencies happen; please contact us as soon as possible if you need to cancel.
Payment Terms
Payment is due at the time of service. We accept major credit cards, debit cards, and cash. Some services may require a deposit at the time of booking. Package purchases are non-refundable but may be transferred to another person with advance notice.
Informed Consent
Before receiving any treatment, you will be required to sign an informed consent form specific to that procedure. This form will explain the nature of the treatment, expected results, potential risks and side effects, and alternative treatments.
Medical History
You are responsible for providing accurate and complete medical history information. Failure to disclose relevant medical conditions, medications, or allergies may affect the safety and effectiveness of your treatment.
Results Disclaimer
Individual results may vary. While we strive to achieve optimal outcomes, we cannot guarantee specific results from any treatment. Results depend on various factors including your individual physiology, adherence to post-treatment instructions, and lifestyle factors.
Liability Limitation
To the fullest extent permitted by Florida law, Medspa Bella, PLLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services.
Dispute Resolution
Any disputes arising from these terms or our services shall first be addressed through good faith negotiation. If negotiation is unsuccessful, disputes shall be resolved through binding arbitration in Pinellas County, Florida, in accordance with the rules of the American Arbitration Association. This arbitration agreement is made pursuant to a transaction involving interstate commerce and shall be governed by the Federal Arbitration Act.
Governing Law
These Terms of Service shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions.
Changes to Terms
We reserve the right to modify these terms at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services after any changes constitutes acceptance of the modified terms.
