Your Rights as a Patient
You have the right to: - Receive respectful care regardless of race, color, religion, sex, national origin, disability, sexual orientation, or source of payment - Be informed about your diagnosis, treatment options, and expected outcomes - Participate in decisions about your care and treatment - Refuse any treatment to the extent permitted by law - Privacy and confidentiality of your medical information - Access your medical records and request amendments - Receive an itemized bill and explanation of charges - Know the qualifications of healthcare providers treating you - Be treated in a safe and clean environment - Voice complaints without fear of retaliation
Your Responsibilities as a Patient
As a patient, you are responsible for: - Providing accurate and complete medical history information - Following the treatment plan agreed upon with your provider - Keeping scheduled appointments or providing timely notice of cancellation - Treating staff and other patients with respect - Paying for services in a timely manner - Asking questions if you do not understand your treatment - Following post-treatment instructions for optimal results
Filing a Complaint
If you have concerns about the care you received or believe your rights have been violated, we encourage you to: 1. Speak directly with your provider or our office manager 2. Submit a written complaint to our Privacy Officer 3. Contact the Florida Department of Health if your concerns are not resolved
